个人简介:
Professor Kazuyoshi Hirota entered Department of Anesthesiology and also Hirosaki University Graduate School of Medicine, April 1986. He received his PhD from Hirosaki University Graduate School of Medicine in March 1990. Subsequently, he became an Instructor of Department of Anesthesiology in Hiroskai University Hospital. He went to University of Illinois at Chicago College of Medicine as a Visiting Assistant Professor of Anesthesiology from July 1993 to June 1994, and also to Leicester University as an Honorary Clinical Fellow of Anaesthesia Department from June 1995 to May 1996. He became an Assistant Professor of Department of Anesthesiology in Hirosaki University Hospital. Then, he promoted to be a Professor and Chairman of Department of Anesthesiology in Hirosaki University Graduate School of Medicine. He has been Vice-Dean of the medical school since February 2012.
He has more than 180 publications of original articles, review article and clinical reports being published in the English peer reviewed journals (e.g. Anesthesiology, British Journal of Anaesthesia, Anesthesia and Analgesia, Anaesthesia, Critical Care Medicine, British Journal of Pharmacology, Naunyn-Schmiedeberg's Archives of Pharmacology, British Journal of Clinical Pharmacology, Neuroscience, Brain Research, Journal of Gastroenterology, Journal of Gastroenterology and Hepatology and etc.) covering research fields of Anesthesiology, Pharmacology, Neuroscience, and Gastroenterology. His main research interests are mechanism of general anesthesia, effects of anesthetic agents and vasoactive agents on airway smooth muscle tone, clinical research regarding total intravenous anesthesia, regional anesthesia, pain and biomarkers, efficacy of perioperative H2 blockers and PPI. His research has been supported mainly with grants-in-aid for scientific research from Ministry of Education, Culture, Sports, Science and Technology and Hirosaki University.
He annually reviews about 100 scientific papers of peer reviewed journals such as British Journal of Anaesthesia, Journal of Anesthesia, JA Clinical Reports, etc because he has been Editorial Board Member of British Journal of Anaesthesia, Journal of Anesthesia (former Editor-in-Chief, now Section Editor), JA Clinical Reports (Editor-in-Chief). He is a grant reviewer for the Ministry of Education, Culture, Sports, Science and Technology in Japan, the National Institute of Academic Anaesthesia in the UK and the Hong Kong Research Grants Council in China. He has been supervising more than 20 PhD/MD students.
He is a Fellow of Royal College of Anaesthetists elected (UK), active member of Association of University Anesthesiologists (USA). He is an Chief of Publication Committee in Japanese Society of Anesthesiologists, Councilor of Japanese Society of Anesthesiologists, Councilor of Japan Society for Clinical Anesthesia, Councilor, Japan Society of Pain Clinicians, Director of Japan Society of Circulation Control in Medicine, Director, Japanese Society for Intravenous Anesthesia, Director of Japanese Society of Regional Anesthesia, and an experts committee member of Research Center for Science Systems in the Japan Society for the Promotion of Science.

麻醉是为保证手术的顺利进行而通过麻醉药物对患者施以镇静、镇痛、催眠、肌松等多方面效用,以消除患者对手术中各种刺激的感知及反应。麻醉剂的浓度是麻醉医生实施安全有效麻醉的依据。在实施全静脉麻醉时的主要麻醉剂是丙泊酚,国内外尚无实时麻醉药浓度监测的设备。气相色谱(GC)、高效液相色谱(HPLC),气质联用(GC-MS)和液质联用(HPLC-MS)虽能监测血药浓度,但不能实时在线测量、耗时较长,均不可能应用于临床。因此,解决生命科学中全身麻醉药代谢动力学及药效学的实时动态测量成为困扰麻醉医生安全精准麻醉的难题。
呼出气静脉麻醉药丙泊酚的浓度监测是2007年才发现的,被誉为世界静脉麻醉的十大发现,其浓度的范围仅在1~30 ppb范围内,静态测量已经相当困难。李海洋教授与哈医大李恩有教授合作利用具有自主知识产权的IMS快速检测技术,研制新型设备,达到静脉麻醉剂丙泊酚呼出气浓度稳定动态测量,实现对其血药浓度、呼出气浓度与脑内摄取浓度的同步测量。广田教授是国际上著名的静脉麻醉专家,他将阐述精准麻醉中目前存在的和亟待解决的科学问题,进而提出开发IMS快速检测技术与研制新型设备的必要性,为最终个性化和精准麻醉的实现奠定坚实的基础,使全身麻醉从经验走向智能化。
患者的麻醉深度取决于中枢神经系统(脑,效应室)内麻醉剂的浓度,而效应室浓度与麻醉剂在血液或血浆中的浓度,即血药浓度相关。精准麻醉的实现,需要了解给药后麻醉剂在效应室及血液中的实时浓度分布和变化趋势,因此,亟待发展实时测量麻醉剂的血药浓度的仪器和方法。全身麻醉中因手术对人体脏器功能影响较大,代谢组学的研究成果能否揭开麻醉中静脉麻醉药的脏器保护机制,目前也未见报道,广田教授从全麻机制研究成果提出代谢组学对围手术期脏器功能保护的可能性和必要性,为广大研究者与麻醉医生之间开创交流的途径,让我们分析化学研究成果转化为麻醉使用的创新技术。
